Patient Information
Please plan to arrive a few minutes ahead of your scheduled appointment. You may be asked to fill out paperwork. The following information needs to be available:
- Your photo ID.
- Your Insurance Card.
- Your Physician’s order or prescription.
- Your medical history. (For certain procedures)
Any co-payment required by your insurance company is due at the time of your visit. For your convenience, we accept cash, personal checks, Visa and Master Card.
Depending upon the procedure or test your physician has ordered, some exam preparation may be necessary. The table below will provide you with any information you may need to prepare for your test.

How To Obtain Medical Records
We automatically furnish your referring physician with a copy of your reports. If you need additional copies, we can fax, mail or courier to authorized facilities.
